We are currently recruiting an experienced Children's Social Worker to join the Children in Care (CIC) Team within Halton Borough Council. This role specifically focuses on supporting young people aged 16 to 18 as they prepare to transition to independence or leaving care services. You will be part of a supportive and established team dedicated to achieving positive outcomes for looked after children and care leavers. Manage a caseload of young people aged 16-18 within the Children in Care pathway. Undertake statutory visits, reviews, and care planning in line with legislation and local procedures. Work collaboratively with Personal Advisors, IROs, education providers, housing, health services, and other professionals to support the transition to independence. Ensure young people are prepared for leaving care by promoting their participation in pathway planning and decision-making. Ensure that safeguarding measures are in place and regularly reviewed. Maintain accurate and up-to-date case records and reports on the local authority's case management system. Qualified Social Worker with a Social Work England registration. ~ Minimum of 2 years post-qualified experience within children's social care, preferably in a CIC or Leaving Care Team. ~3 years perm experience. ~ Enhanced DBS on the Update Service or willingness to apply for one.
