In this vital role, you will:Deliver a specialist Occupational Therapy service, identifying options to enhance independence and reduce reliance on care. Use a wide range of tools, including equipment, assistive technology, manual handling techniques, and training to support clients.
Lead on early reviews of post-hospital discharge packages, helping reduce long-term care needs and ensuring effective use of care resources. What we're looking for: ? HCPC registered Occupational Therapist ?
Experience in community-based or hospital discharge services ? Strong knowledge of equipment provision, risk assessment, and reablement strategies ? Ability to work collaboratively with health and social care professionals
